Health Care Reform 

The Patient Protection and Affordable Care Act (commonly called Health Care Reform) has generated notice requirements in 2014 with which all employers must comply. We are required to provide all employees with a notice entitled New Health Insurance Marketplace Coverage Options and Your Health Coverage.  

We are required to notify you of other coverage options available in the Health Insurance Marketplace. LPSS has no information or expertise on the marketplace options, and the Insurance Department staff are not allowed to answer questions regarding the options. To find answers, employees will need to consult with the resources identified in the attached notice.

As your employer, LPSS is required by law to obtain a Wavier of Coverage form should you choose not to elect our employer health care insurance. This form requires a reason why, a date, and a signature. Forms are to be returned to LPSS Insurance at Central Office via e-mail, pony mail or in person at 113 Chaplin Drive.
